Constantine has the ability to see the "true face" of supernatural or magical beings, and/or psychically pick up things of a similar nature. In Thedas, he still has this ability, and can see persons/creatures possessed by demons or spirits, as well as possibly pick up any hints as to the "true form" of Rifters who were humanised on arrival to Thedas. What is their supernatural/magical nature? Before his qunarisation, Loxley was a tiefling. Kind of fun here for the fact that this is, in fact, a real devil-adjacent thing to be.
How would this present? His true form is golden skin, grey-gold horns, black/gold eyes, devil tail. More abstract associations could be the hissing sound of the infernal language he knows from birth, the smell of brimstone, etc. I don't quite remember how it works for Keanu but I'm happy for you to riff off of any of that.
